What Is Superficial Gluteal Lipofilling?

Superficial Gluteal Lipofilling (SGL) Is The New Version of BBL

Superficial Gluteal Lipofilling, also referred to as gluteal fat grafting, is the  transferring of autologous fat from body areas to be injected above the muscle layer. This is the only technique the CREO Clinic uses in its Brazilian Butt Lift procedures, as traditional intramuscular fat transfer and buttock implants have proven to be far riskier and less effective. 

Fat Grafting

The process of fat transfer involves using liposuction to extract fat from areas of the body where it is in excess, purifying it, and then injecting it under the skin wherever the patient wants to see increased volume and enhancements in shape. In the case of a buttock augmentation, the fat transfer is injected above the muscles of the buttocks and hips.

Ultrasound Scanning Guides the Process

Dr. Tillo uses ultrasound scanning to give him accurate images of the anatomy below your skin to ensure that no fat enters the muscle. This allows him to guide the injection and ensure the fat injection is subcutaneous but always above the gluteal muscle.

Superficial Gluteal Lipofilling Is The Safest BBL

These guidelines underscore the significance of providing meticulous, high quality and safe care and outcomes.

Dr. Tillo describes the history of BBL surgeries and why his technique is superior: 

“In the past, when surgeons performed Brazilian Butt Lift (BBL) surgery, they used to inject fat deep into the buttock muscles. However, this method had serious risks, including fat leaking into large blood vessels, leading to a condition called fat embolism, and in many cases, it even resulted in death.

To make the procedure safer, I developed a different technique called BEST-F (Buttock Enhancement with Superficial Transfer of Fat). Instead of injecting fat deep into the muscles, I placed it just under the surface. I also used an ultrasound scanner during surgery to carefully track the cannula’s position, ensuring that the fat went where it was supposed to and not into the muscle.

My experience and knowledge in this area led me to be part of the BAAPS Gluteal Fat Transfer Review group, and I co-authored national guidelines for BBL surgery which was published in 2023. Unfortunately, not all surgeons, especially those outside the UK, are aware or follow these safety recommendations. Some still inject fat blindly or deliberately into the muscle to achieve very large changes in the buttock shape, which can be dangerous.

It’s crucial for everyone’s safety that surgeons prioritise safety guidelines when performing BBL surgery. Injecting fat into the muscle can have serious consequences, and it’s important that surgeons take steps to prevent these risks for the well-being of their patients.”

Ideal Candidates for SGL

To be suitable for an SGL surgery, patients must:

  • want to enhance the shape and contour of the hips or buttocks
  • have realistic expectations of the surgical procedure and its outcomes

  • be in good physical and mental health

Low BMI

Having a suitable BMI below 30 before undergoing a BBL surgery, as this minimises the risk of complications. Some patients may have a low BMI and prefer to undergo a skinny BBL, a surgery designed specifically to add volume to the butt and hips of slimmer patients with low BMIs.
