What Is MTF Top Surgery?

Male-to-Female Top Surgery is a type of breast augmentation procedure using implants, fat transfer or a mixture of both surgical techniques to create a more feminine chest by increasing the volume and moulding the shape of the breasts. 

It can be used in addition to hormone therapy, building on breast tissue developed from this treatment, or in the absence of hormones to create a curvier upper body that is traditionally feminine in appearance.

Who is a Candidate for MTF Top Surgery?

During consultation with Dr. Tillo, your suitability for MTF top surgery will be discussed to determine whether the procedure is right for you.

In general, you are a good candidate for MTF top surgery if:

  • You are a trans woman who has been diagnosed with gender dysphoria
  • You have been living in your preferred gender for at least one year and have undergone psychological assessment
  • You are over 18
  • You are in overall good physical and mental health, with a suitable BMI under 30
  • You wish to enhance the size and shape of your breasts to achieve a more feminine aesthetic

Male to Female Top Surgery Procedure

Breast augmentation for transgender patients is typically performed under general anaesthetic, and takes 2-3 hours to complete. 

For a breast augmentation using implants, an incision is made under the creases of the breasts, beneath the nipples or in the underarm, depending on whether silicone implants or saline implants are being used. As the breast tissue is likely to be less developed in transgender patients, the implants are usually placed under the muscle to provide better coverage and reduce the risk of rippling. The incisions are then closed using sutures. 

When fat transfer is used for male to female top surgery, liposuction is required to remove fat from parts of the body where it is in excess. This extracted fat is then purified and directly injected into the breasts to achieve the desired shape via a series of small incisions. 

Recovery

You may experience swelling, bruising and tightness around the breasts following male to female breast surgery. These side effects typically dissipate within 1-2 weeks, after which you should feel comfortable carrying out light, daily activities and returning to work. However, you should wait until you are fully recovered before exercising and carrying out more strenuous activities, which typically takes 6 weeks.

During these first six weeks, you will need to wear a compression vest to manage any swelling, which can help to alleviate any associated discomfort. 

What Are the Potential Risks of MTF Top Surgery?

Like any surgical procedure, MTF top surgery carries a degree of risk though complications are rare. General risks include bruising, swelling, infection, scarring and bleeding. 

Risks of Inserting Breast Implants: 

  • Loss of nipple sensation
  • Capsular contraction
  • Implant rotation, loss or rupture
  • Aesthetic concerns and asymmetry e.g. changs in shape, rippling, creasing or folding
  • Anaplastic Large Cell Lymphoma (rare)
  • Breast implant illness

Risks of Fat Transfer Breast Augmentation:

  • Volume loss requiring repeated procedures
  • Aesthetic concerns and asymmetry e.g. lumpiness, waviness, skin discolouration, wrinkling, skin laxity or dimpling
  • Fat necrosis

What to Expect Before and After the Procedure

Below we have outlined the general step-by-step process for MTF top surgery at the CREO Clinic:

Consultation

During your initial consultation, Dr. Tillo will discuss your medical history with you to determine your suitability for surgery. You will discuss your expectations for top surgery, and the realistic result that the procedure can provide to ensure that they are aligned.

Leading Up to the Procedure

In the lead up to surgery, you will need to make several preparations:

  • Maintain a stable weight, with a healthy BMI under 30. 
  • Four weeks prior to surgery, you will need to stop any hormonal therapy as this can cause complications. 
  • Stop nicotine consumption for 4 weeks before and after surgery to reduce the risk of complications.
  • Cut down your alcohol intake.
  • Prepare your home for recovery and make plans for child and/or pet care during the initial stages of healing. 

Day of the Surgery

On the day of surgery, you will need to arrive at the allotted admission time provided by the team to complete any necessary pre-surgical checks and allow time for paperwork to be completed. 

Dr. Tillo will mark the area where fat is to be taken and injected, and/or where implants are to be inserted. He will discuss your treatment plan with you and you will sign a consent form for surgery. 

You will also speak to the anaesthetist to determine your suitability for general anaesthetic before this is administered. 

Treatment and Recovery

MTF top surgery is a day case procedure, and typically takes 2-3 hours to complete.

The surgical techniques employed and incisions made will depend on the type of breast augmentation chosen – implants, fat transfer or a composite breast augmentation. 

After surgery, you will be taken to the recovery room where you will gradually wake up from your anaesthetic. Prior to leaving the hospital, you must pass urine and have something to eat or drink. You can then be discharged by the team to an adult who must stay with you for the first 24 hours after surgery. You may also be required to wear a surgical bra for a period of time after the surgery.

Additional Physique Adjustments 

Buttock Augmentation

In addition to MTF breast surgery, many trans women choose to undergo additional procedures to feminise the figure. A buttock augmentation using either implants, fat transfer or a combination of both, can create a curvier, highly desirable figure that is traditionally feminine. This procedure can help to even out the proportions of the lower body after the breasts are augmented.

Is Top Surgery Painful?

As male-to-female top surgery is performed under general anaesthetic, you will be asleep for the duration of surgery. After you wake up, you may experience some soreness and tenderness around the chest, though Dr. Tillo will provide you with any necessary pain relief to manage your discomfort as your body heals. <br><br>In general, most patients find that the discomfort from MTF top surgery gradually dissipates within 1-2 weeks, after which you should feel comfortable enough to return to office work and light, daily activities.

How to Minimise Scarring after MTF Top Surgery?

You will be given comprehensive aftercare guidelines by Dr. Tillo to minimise the appearance of scarring after surgery. You should avoid smoking, sun exposure, excess strain on the incision sites and scratching as these factors can increase the prominence of scars. <br><br>Overall, your scars will gradually fade over time as your body heals.

Can you Get MTF Breast Augmentation Without Hormones?

Yes – hormone therapy is not required to undergo a MTF breast augmentation procedure. However, it is preferable that patients have been undergoing hormone therapy for at least a year, as this can create additional volume in the chest with their own natural breast tissue.

How do you Shower After Top Surgery?

You will be able to shower 48 hours after top surgery. You should avoid bathing or soaking the incision sites for the first 6 weeks after surgery, as this can impede the healing process. Instead, keep showers short, use lukewarm water and gently pat or air dry the chest.
